Report Retention Change for MDS 3.0 Facility-Level Quality Measure and MDS 3.0 Resident-Level Quality Measure Preview Reports (posted 04/21/2016)

The retention time period for the MDS 3.0 Facility-Level Quality Measure and MDS 3.0 Resident-Level Quality Measure Preview reports will be changing. These automatically-created preview reports are currently stored in each nursing home's shared facility folder for a period of 230 days. Effective November 1, 2016, the above mentioned preview reports will only be stored in the shared facility folder for a period of 90 days.

The report retention time period change affects the following:

  1. Any new MDS 3.0 Facility-Level Quality Measure and Resident-Level Quality Measure Preview reports saved into each nursing home's shared facility folder after November 1, 2016. These new reports will be retained for a period of 90 days following the date the report was added to the folder.
  2. Any existing MDS 3.0 Facility-Level Quality Measure and MDS 3.0 Resident-Level Quality Measure Preview reports with a create date older than 90 days prior to November 1, 2016 will be automatically deleted from the system.

Action: Facilities are encouraged to print or save a copy of the MDS 3.0 Facility-Level Quality Measure and MDS 3.0 Resident-Level Quality Measure Preview reports prior to November 1, 2016. Any reports not printed or saved prior to the retention period time change will be permanently deleted from the facility shared folder if the date the report was added to the folder is older than 90 days.

Note: These reports cannot be recreated once they have been deleted. The report retention time period change does not affect the MDS 3.0 Five Star Preview reports.

Notice: Five Star Preview Reports (posted 04/19/2016)

The Five Star Preview Reports will be available on April 20th. To access these reports, select the CASPER Reporting link located on the CMS QIES Systems for Providers page. Once in the CASPER Reporting system, select the 'Folders' button and access the Five Star Report in your 'st LTC facid' folder, where st is the 2-character postal code of the state in which your facility is located and facid is the state-assigned Facility ID of your facility.

Nursing Home Compare will update with March's Five Star data on April 27, 2016.

Important Note: The 5 Star Help line (800-839-9290) will be available from April 25, 2016 through April 29, 2016. Provider preview reports will continue to be available on a monthly basis in advance of public posting and will include the dates and hours of helpline availability. BetterCare@cms.hhs.gov is an alternative communication medium to direct inquiries.


PBJ Data Submission Specifications Errata V2.00.1 for PBJ Data Submission Specifications V2.00.0 (posted 03/30/2016)

An updated errata V2.00.1 has been posted for the PBJ Data Submission Specifications V2.00.0, which are scheduled to go into effect on June 27, 2016. The only impact of this errata document to XML submitters is the size of the XML file. Nothing has changed with regard to XML format, other than what was already communicated in the V2.00.0 PBJ Data Submission Specifications, which were posted in January, 2016. Six items have been identified in the errata, including updates to the data specifications and the data specification overview document.

MDS System-Generated Final Validation Reports Available in Text and XML Format (posted: 09/22/2015)

Two final validation report links display in the Validation Report (VR) folder for each successfully processed submission file. Nursing home and swing bed users should select the final validation report name link that displays adjacent to the green notebook icon to view the validation report in text. The text format is the same format that providers are accustomed to receiving. The XML formatted report is easily identified by the 'XML' reference on the icon and is used by software vendors that develop MDS 3.0 data entry software.

jRAVEN (posted 01/18/2011)

jRAVEN is the new data entry software, which Nursing Homes and Swing Bed providers may use to collect and maintain the MDS 3.0 assessment, resident and facility data to create MDS 3.0 submission files. jRAVEN is now available for download on the jRAVEN / RAVEN page.

Note: Please continue to regularly check the QTSO Web site for software updates or additional information.

The RAVEN 8.3 and RAVEN-SB 2.0 software should continue to be used for the MDS 2.0 system. Do NOT uninstall or remove this software.


Notice: MDS Personal Login ID Requirement (updated 02/21/2010)

A MDS Personal Login ID is required in order to access the MDS Submission system and CASPER Reports. In order to acquire a Personal Login ID, a request must be submitted to the Help Desk. A link to the MDS Personal Login ID forms can be found on the Data Access Request Forms page https://www.qtso.com/accessmds.html. Forms will be processed within 5 business days.

After you receive your new MDS personal login ID, login to the QIES User Maintenance Application using the link on the MDS State Welcome Page. When you login you will enter answers to security questions, as well as, create a personal, secure password. The answers you enter for the security questions can be used to securely identify you in the event that you need to reset a forgotten password or reset an inactive account.
